1、 What documents do newborns need to apply for
1. Birth medical certificate. When expectant mothers are admitted to the hospital, the hospital will require them to fill out the "Birth Medical Certificate Self filling Form". After the baby is born, the hospital will issue a "Birth Medical Certificate" based on this.
2. Newborn household registration. It is best for the parents or guardians of the baby to apply for birth registration with the local police station where the baby's father or mother resides, within one month after birth, by presenting the "Birth Medical Certificate", "Birth Medical Certificate Copy", the baby's parents' "Marriage Certificate" and "Household Registration Book".
3. Medical insurance card. Go to the community where you live to get the Registration Form of Basic Medical Insurance for Urban Residents, fill in relevant information, and bring your household registration book and photocopy with you. The original and photocopy of your household registration book, as well as your ID card and photocopy/ID card number should be brought with you.
4. Vaccination certificate. When the baby is born within one month, he/she should take the first dose of hepatitis B vaccine and BCG vaccination record certificate from the baby's delivery room and go to the disease prevention and control center in the jurisdiction where his/her household registration is located to apply for a child vaccination certificate, so as to timely vaccinate the second dose of hepatitis B vaccine and other corresponding vaccines.
5. ID card. Children's ID cards or child ID cards must be accompanied by parents to apply for, and children must wear a dark collar top with a validity period of 5 years. Bring your parents' ID card, children's household register, birth certificate, and school registration certificate (not necessary for those who have not attended school).
